You may specify values of your telephone area code, and (for U.S. addresses) city, state, and zip code. If you do, these values will be used if you leave the corresponding fields blank further down the form. They will be used for up to 10 telephone numbers and two street addresses.

A college or university is either a public or a private institution. If this information is not known, it will be added by the ECEDHA office staff.

Different universities use different titles for the chief executive of the department. Select one of the titles in the pop-up menu or else select Other and type the title in the space provided. This field is required.

If you have an IEEE Student Branch, select one of the titles in the pop-up menu or else select Other and type the title in the space provided. This field is optional; if not present the entire section is omitted.
IEEE divides the planet into ten regions for administrative purposes. The region (if known) can be entered by the submitter. If it is missing, it will be added by the ECEDHA office staff.

If you have a chapter of Eta Kappa Nu, select one of the titles in the pop-up menu or else select Other and type the title in the space provided. This field is optional; if not present the entire section is omitted.
In most universities the department is part of a larger unit, often called the School of Engineering, headed by a Dean of Engineering. The titles used vary. If you have such a unit, select one of the titles in the pop-up menu or else select Other and type the title in the space provided. This field is optional; if not present the entire section is omitted.
The title of the chief executive of this unit varies, although it is usually something like "Dean of Engineering." Select one of the titles in the pop-up menu or else select Other and type the title in the space provided. This field is optional; if not present information about this person is omitted.
Space is provided for the URL of the World Wide Web home page of the university, the department, the school, the IEEE Student Branch, and the Eta Kappa Nu chapter. Space is also provided for the URLs of personal home pages of all people named in the entry.
URLs begin with http:// and then include the name of the computer, and the path starting at the root directory of the computer down to the location of the HTML file. Each URL present in the form will be used in an active link in the formatted entry, for the person or the unit in question.
Space is provided for the mailing address of the department and the school. For U.S. addresses, put the city, state, or zip code in the space provided, rather than in the box marked street address. For non-U.S. addresses, ignore the special places provided, and put the complete address, including city, postal code, and country as a line in the mailing address. You may use the abbreviations customarily used in mailing addresses. The city, state, and zip code need not be entered other places if they are entered as general values at the top of the form.
Space is provided for the telephone numbers of all the people named, as well as for the department and the school. Space is also provided for the department and school fax numbers. For phones and faxes in North America, the country code is 1, and should be entered at the top of the form. The three-digit area code need not be entered other places if it is entered as a general value at the top of the form.
For phones outside the North American Numbering Plan, enter the country code in the general-values area at the top of the form, and in place of the area code use whatever subsequent city codes are required.
Note that the office phone numbers need not be the same as the personal phone numbers.
Space is provided for the personal e-mail addresses of all the people named, as well as office e-mail addresses for the department and the school. There is also a place for an e-mail address for people to use in requesting information about graduate school, so that the Graduate Coordinator is spared the many inquiries. Note that the office e-mail addresses need not be the same as the personal e-mail addresses.
These addresses should all be in the standard Internet form, name@node, where the node is a fully qualified name probably ending in .edu.
There is space for names of the people relating to the department, including the department head, the graduate coordinator, and so on, and also for the Dean. There are actually three fields for each name: one for the first name including middle initial or name, one for the last name (family name), and one for a suffix such as "Jr." or "III".
Check the box next to each of the types of degree programs your department offers. Check to see whether your program is ABET-accredited as an Electrical (or similarly named) program or a Computer (or similarly named) program (or perhaps you offer both).
The term "Basic Level" refers to four-year programs, and "Advanced Level" to programs that typically lead to an advanced degree. Normally ABET does not accredit two programs in the same field at one institution, so if the basic program is accredited then the advanced program is not.
This information (if known) can be entered by the submitter. If it is missing, it will be added by the ECEDHA office staff.
Please identify yourself as the submitter by including your name, personal e-mail address, and phone number. This information will enable the ECEDHA office staff to contact you in case there are any questions. The e-mail address you give here will be used to send you confirmation of your Save, Submit, and Delete actions.
Use this space to send a message to the ECEDHA office staff regarding this entry. In particular, if you request that the entry be deleted, here is where you should explain why. It would also be helpful if you could indicate briefly what changes you have made in the entry, e.g., "We have a new Graduate Counselor," or "I have added the department URL," or some such.
This space is used by the ECEDHA office staff to send a message back to the submitter. This is useful in case a submitted entry is returned for further editing.
You can use this form to edit your entry in a way similar to how you use a word processor. You can select any of four action buttons (plus two more for the ECEDHA office staff) to control what you want to happen.
